Your First Appointment

What to do:

  • Contact your insurance company to determine if our services are covered and if you need a referral from your primary care provider. Please get this referral prior to your first visit.
  • Please arrive 30 minutes before your scheduled appointment time to complete information forms.
  • Please bring all insurance cards, a photo ID, and a current list of medications. Inform our medical staff of any known allergies.
  • If you are unable to keep your appointment, please notify our office 24 hours before your appointment.

Our Policies

Payment Policy/Insurance Plans

Medical Insurance is an important part of everyone’s health care today. Since there are so many types of insurance plans available, coverage can be very different depending on which plan you are covered under.

It is a good idea for you to understand your current insurance plan. Some plans require that you get approval from your regular doctor to see a specialist. This is called a referral. If this is how your plan is set up, then you will need to get the referral prior to your scheduled appointment with a Urology Specialist.

Our Urology Practice participates with many different Insurance plans; the most common plans are listed below.

Please have your insurance card with you at the time of your appointment. You will be asked to show your card at each visit. If there is a co-payment for service, your will need to pay this at the time of the visit. If your insurance company does not cover our services, or if we are not a participating provider with your insurance carrier, payment will be expected in full at the time of your visit. We accept cash, check, and credit cards such as Visa, Master Card, or Discover.

After-Hours & Prescription Refills

After-hours services are available for patient emergencies. To reach the appropriate on-call physician for an emergency, call the office you usually visit and follow the recorded instructions. We accept calls for prescription refills during regular office hours (8:30 a.m. to 5 p.m., Monday through Friday). When you call, please provide the prescription name, pharmacy name, and pharmacy phone number. Unfortunately, prescriptions cannot be filled in the evening or on weekends because we must have access to your medical chart before medication is prescribed.

Electronic Medical Records

We use electronic medical records (EMR) to quickly and efficiently preserve critical patient medical information, which greatly assists with assessment, diagnosis and treatment decisions. Using an electronic system, rather than cumbersome paper-based records, means physicians’ orders can be issued quickly and patient information captured immediately. Our streamlined system means our urologists can quickly access results of lab tests, lists of current medications, and physician notes.
